Admission Criteria
For nearly twenty years, On With Life at Ankeny has provided specialized inpatient rehabilitation services to individuals aged 10 and up. In order to receive our services at On With Life at Ankeny, our person served must:
● have acquired a brain injury or other neurological disorder
● have the potential to participate in and benefit from comprehensive rehabilitation services
● have the potential to participate in and benefit from comprehensive rehabilitation services
● are medically stable and no longer require acute hospitalization
● breathe independently, without medical assistance
● do not demonstrate a severe behavioral, psychiatric or personality disorder which poses a threat to self or others, or that requires intensive or long-term psychiatricintervention
● have means of payment
● if either minors or adults are not capable of informed decision making, have either a legally appointed decision maker or a family representative responsible for decisions and/or informed choices.

● do not demonstrate a severe behavioral, psychiatric or personality disorder which poses a threat to self or others, or that requires intensive or long-term psychiatricintervention
● have means of payment

Discharge Criteria
The person served:
• has improved sufficiently so he or she no longer needs the services provided within the inpatient rehabilitation program.
• is not medically stable and can no longer benefit from the intense inpatient rehabilitation program.
• responsible party determines that he or she no longer wants to participate in the comprehensive inpatient rehabilitation program.
• no longer has a viable funding source for inpatient rehabilitation services.
• has the means to pay but has failed, after reasonable and appropriate notice, to pay for services.
