Creating a Community of Care
On With Life's Supported Community Living Program was created in 1998 to develop and improve self confidence and skills required for community living, self advocacy and meaningful social community relationships for persons with brain injury. To enhance this process, ongoing education and supports are also provided to the family and other members of the individual's support system.Our staff members collaborate with the person served, the family/support system and an external Case Manager to design a coordinated service plan focused on maximizing personal strengths. Ongoing meetings with all the stakeholders are conducted to chart progress and plan strategies to minimize barriers.
Our services are provided to individuals in their homes throughout communities in Polk, Warren, Jasper, Story and Dallas Counties in Central Iowa.
Joining hands, hearts, and minds
Our goal is to provide specialized and culturally sensitive support to allow persons who have experienced a brain injury or other neurological impairment to maintain and/or improve their independence. Referrals to schools, employers, training programs, Iowa Vocational Rehabilitation Services and places of worship are made to help address educational, vocational, cultural and spiritual needs.Accreditations
The Bureau of Long Term Care of the Department of Human Services as a "Home and Community Based Waiver Provider."
