Keynote Speaker

Dr. Leif Leaf, Ph.D., is a licensed psychologist specializing in Clinical Neuropsychology, Rehabilitation Psychology and Family Counseling.  He has more than 25 years of experience working with individuals and families who have experienced traumatic brain injury and rehabilitation needs and has served as a Surveyor for the Commission on Accreditation of Rehabilitation Facilities for the past 20 years.

CEU Information

  • Nurses: This program has been submitted for approval to Mercy Medical Center, Des Moines, Iowa Board of Nursing #17 for 7.2 contact hours.
  • This program has been submitted for approval to The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. The course has been submitted for 6.0 CE contact hours. 
  • This program has been submitted for approval to University of Iowa Rehabilitation & Mental Health Counseling Program for 6.0 contact hours for CRCs.

  • All professionals: All registered attendees will receive a certificate of attendance from On With Life stating CEU credits earned. Certificates will be sent (electronically) following the program and will contain pertinent conference information for verification to licensure boards for review and approval.

  • CEUs provided only for complete attendance, no partial credit awarded. CEU documentation will be mailed.  

Registration Costs

Registration costs include continental breakfast, lunch and snacks. An additional fee will be charged for those registering for nursing, case management and CRC credits to cover admin costs and CEU fees charged by these accrediting organizations. All professional registration attendees will receive a certificate of attendance from On With Life stating CEU credits earned (at no additional charge). Certificates will be sent electronically following the program and will contain pertinent conference information for verification to licensure boards for review and approval.

  • Professional Registration - Early bird rate: $65 (includes therapists, social workers, SCL specialists, etc.). $75 after August 26
  • Additional CEU Fees: 
      • Nursing - $15
      • Case Management - $10
      • Certified Rehab Counselor (CRC) - $5
