Fall Prevention Clinic

On With Life's Outpatient Neuro Rehabilitation Program would like to introduce you to our comprehensive falls prevention clinic, an innovative program designed for individuals who have experienced, or are at risk for falls. The program seeks to decrease the risk of injury, improve independence and increase the quality of life.


Our program includes a comprehensive assessment and unique treatment course designed specifically to address medical complications, strength, vision, balance, coordination and cognition. In addition, each individual receives customized recommendations to maximize safety and independence specific to their fall-related risk factors.

Our Clinic

Your initial visit may include an appointment with an internal medicine physician or nurse practitioner who will evaluate your medical status and identify necessary specialty services (required for all self-referrals).

If recommended by the provider, you will then be assessed by our specialty trained physical and occupational therapists. Our team will identify risk areas that place you at higher likelihood for falls and develop specific interventions to address those risks. Once the comprehensive assessment is complete, an individualized therapy program will be developed to address the goals you have identified. The typical course of treatment lasts 4 to 6 weeks.

Our Approach

Our comprehensive fall prevention team will provide you with a combination of the following:

  • A thorough medical evaluation and medication review by an internal medicine physician or nurse practitioner

  • A comprehensive physical and occupational therapy evaluation to address balance, vision, strength, coordination, cognition and flexibility

  • Therapy sessions provided by experts in the field of neuro-rehabilitation

  • Specialized equipment including the Biodex┬« balance system and Dynavision┬« visual-motor and cognitive skills system

  • A vestibular (balance system) screening, with in-depth evaluation by a certified vestibular specialist as needed

  • A personalized balance and fall-related plan with recommendations for an at-home program

  • Strategies to help you create a safe environment, decrease your fall risk factors and implement healthy lifestyle changes

  • Functional visual assessment, strategies and recommendations

  • Information on community resources and programs to help you maintain your goal of staying falls-free

Who is the Falls Prevention Clinic for?

Individuals who have one or more of the following risk factors may be appropriate for the Falls Prevention Clinic: 

  • History of Falls
  • Neurological conditions such as Parkinson's disease or multiple sclerosis
  • Gait disorder, use of an assistive device
  • Balance disorder
  • Muscle Weakness
  • Arthritis
  • Visual deficits
  • Neurologic deficits
  • Impaired Activities of Daily Living (dressing, hygiene, feeding, etc)
  • Depression
  • Cognitive impairment
  • Over 80 years old
  • More than four medications
  • Cardiovascular deficits
  • History of a head injury or brain injury

Referral Process

Self-referrals can be made by calling our office at 515-289-9696. If the referral is through a provider, your provider can fax prescription, history and physical information with a referral to On With Life Outpatient at 515-289-9649.

Insurance Guidelines

The Fall Prevention Clinic is covered by almost all insurance plans including Medicare, Medicaid and private insurance. We will work with you to secure appropriate documentation and pre-authorization, as needed.
