Post-Acute Inpatient Rehab

On With Life's Post-Acute Inpatient Rehabilitation program is a 28-bed, CARF accredited program that provides intense therapy to those that have sustained a traumatic brain injury, stroke, tumor, loss of oxygen or other neurological condition. On With Life is a unique one-of-a-kind facility. Our extensive rehabilitation experience and our expertise in the treatment of individuals with brain injury offer individuals the optimal opportunity to achieve their goals. 


Individualized, Collaborative Care 

As specialists in brain injury rehabilitation, we know that each injury is unique, with specific needs, capabilities and potential. Therefore, each person served has a treatment plan that is created uniquely for them. The entire rehabilitation team at On With Life works together with the person served and family to develop a treatment plan that meets the goals of those involved. Goals and outcomes are measured on a regular basis and on average, 75% of individual long-term goals are met and 60% of persons served return home at discharge.


Brain Injury Rehabilitation Program

The transdisciplinary team(s) at On With Life works together with the person served and family to develop a treatment plan that meets the goals of those involved. Goals and outcomes are measured on a regular basis and on average, 75% of individual long-term goals are met and 60% of persons served return home at discharge. Average length of stay for this population is 90 days.

Disorders of Consciousness Program

The On With Life Disorders of Consciousness (DOC) program is a unique level of care that has been developed to address the needs of adolescents and adults with severe brain injury who present with low levels of consciousness. This program involves intensive rehabilitation nursing services and specialized physical interventions to address body positioning, muscle tone and medical stability. A specially trained transdisciplinary team provides environmental stimuli, monitors for signs of arousal/awareness and adjusts the stimuli in order to maximize the survivor's improvement. On With Life has admitted more than 250 individuals into the DOC program and of those, 76% emerged from their DOC and approximately 60% of individuals return home at discharge. Average length of stay for this population is 162 days.  

Stroke ENGAGE Program 

The Stroke ENGAGE Program provides medical and rehabilitation care with staff trained to identify and respond to the special needs of individuals recovering from a stroke. ENGAGE is an acronym designed to specify specific program participation requirements:
E - Energy to meet program demands 
N - Neurologically stable  
G - Goals specific to rehabilitation 
A - Access to family and community support 
G - Good control of medical conditions to prevent secondary stroke 
E - Enthusiasm for the rehab process

This program addresses stroke related impairments. Additionally, there is an emphasis on prevention of secondary stroke by assisting persons served and family members to understand the risks of secondary stroke and the behaviors/activities that may increase such risk. The focus of the program is to identify each participant's personal goals and needs. Prioritizing interventions in a manner designed to avoid overwhelming or overstimulation of the person served. Approximately 60% of all On With Life admissions have a primary diagnosis of stroke. Average length of stay for this population is 72 days for traditional brain injury rehabilitation program and 91 days for the Disorders of Consciousness program.


Neuro

On With Life serves individuals that have other neurological conditions, such as West Nile Virus or Guillian-Barre Syndrome. In FY18, 16% of On With Life admissions had a primary diagnosis of Other Neuro. Average length of stay for this population is 80 days for traditional brain injury rehabilitation program and 166 days for the Disorders of Consciousness program. 


Transdisciplinary Team

Nursing services are provided 24 hours a day, seven days a week under the direction of a team of PMR, internal medicine and psychiatric physicians. An advanced practice nurse practitioner is also an integral part of the medical team. Nursing hours per patient per day average well above 8, significantly above standard nursing hours and on par, or above, hospital nursing hours on a typical medical surgical unit. Skilled therapies are provided by a transdisciplinary team to all patients Monday through Saturday and Sundays as needed. Therapy schedules run from 7:30 AM - 5:00 PM with evening activities interspersed during the week. All inpatient persons served are scheduled to receive a minimum of three hours of therapy, five to six days per week. Population Served and Outcomes

On With Life serves individuals across the country. Last year, we served individuals from 37 different counties. The average age of the person served was 51; 63% of our persons served are male and 37% are female.
