Long-Term Admissions

Thank you for your interest in our Long-Term Skilled Care Program for Youth and Younger Adults. Long-term skilled care is the best option for persons living with brain injury who are medically stable, but require intensive nursing, therapy and medical supports that may not be available in other settings. The persons we serve in our program include those who:

  • Are age 10 or older

  • Have experienced a brain injury or other neurological condition

  • Require skilled nursing services

  • Breathe independently, without medical assistance

  • Are medically stable and do not require daily doctor visits or ongoing hospitalization

  • Do not demonstrate a severe behavioral or personality disorder which poses a threat to self or others, or requires intensive psychiatric intervention

  • Has a means of payment for services

Referrals and Admissions

Referrals or inquiries for a Long-Term program admission can be made by anyone with a need, including a physician, discharge planner, social worker, case manager, attorney, the patient themselves or a patient's family member. Please call us at 712-525-1294 or use our online Refer to Us Form and select "Long-Term Skilled Care" as the program option. Once we receive your information, a member of our team will be in contact to gather information and discuss next steps.
