Outpatient Admissions and Program Criteria

Thank you for your interest in our Outpatient Neuro Rehabilitation program. Our specialty outpatient therapy program, located in Ankeny, Iowa (just north of the Des Moines metro) includes neuro-based physical therapy, occupational therapy and speech therapy and our team has one goal in mind, to return you as fully as possible to the activities, passions, roles and talents that define you as an individual.

Admission Criteria

The person to be served:

  • Has acquired a brain injury or other neurological disorder. Persons who also present with a spinal cord injury may be considered on a case-by-case basis

  • Has the potential to participate in, and benefit from rehabilitation services

  • Has a physician order for therapy

  • Is medically stable

  • Breathes independently, without mechanical assistance

  • Does not demonstrate a severe behavioral, psychiatric or personality disorder that poses a threat to self or others, or that requires intensive or long-term psychiatric intervention

  • Has means of payment for services

  • Who is either a minor or adult not capable of informed decision making, has either a legally appointed decision maker or a family representative responsible for decisions and/or informed choices

  • Has a means of payment for services

  • Is age 10 or older

Discharge Criteria

  • The person served has improved sufficiently so he/she no longer needs the services provided

  • The person served is not medically stable

  • A responsible party determines that he/she no longer wishes to participate in the outpatient program

  • There is no viable funding source

  • The person served has the means to pay but has failed, after reasonable and appropriate notice, to pay for services

  • Two instances of no call/no show

Referrals or inquiries for our Outpatient program can be made by anyone with a need, including a physician, discharge planner, social worker, case manager, attorney, the patient themselves or a patient's family member. Please call us at 515-289-9696 or use our online Refer to Us Form and select "Outpatient Neuro Rehabilitation" as the program option. Once we receive your information, our clinical office assistant will be in contact to gather information and discuss next steps.
