What is On With Life at New Horizons Center?

On With Life at New Horizons Center is an adult day center that provides recreational therapy through social interaction and engagement. We serve individuals with physical and/or cognitive challenges, as well as seniors who cannot be safely left alone, demonstrate a decline in cognitive functioning, or are in need of increased socialization.

When are you open?

Our hours of operation are Monday – Friday, 8:00 am – 4:30 pm

How long have you been open?

The original New Horizons Adult Day Center opened in January 2000 with two participants, two staff members and one volunteer. Over the years the program continued to grow and in 2024 officially became a part of On With Life’s continuum as a way to meet a growing need for day services in the community. The program transitioned into On With Life at New Horizons Center on July 1, 2024.

What does a typical day look like?

We provide daily recreational activities to assist with maintaining and/or enhancing physical, cognitive and emotional well-being of our persons served. We often host various groups to the center (such as the Blank Park Zoo or local singing groups) and schedule monthly outings into the community. Each day also includes exercise and a nutritious lunch for persons served. You can find a monthly activity/event calendar and menu on the main day services webpage.

Can I drop off my loved one whenever I need?

We require persons served to attend the center on a regular, scheduled basis. This not only develops a routine for your loved one, but it helps them establish a relationship with our staff and other persons served in the program. Attendance can be every day or a few times a week. You may request to change your loved one’s scheduled day(s) based on availability and needs of the person served. Please note that a 24-hour notice is required for cancellation.

How do I enroll my loved one into this program?

Once you have referred to On With Life’s day services programming, a member of our team will be in contact regarding next steps. These next steps include a tour of the facility where you will learn more about the program and admission criteria, followed by a trial day if criteria is met. Admission will be assessed after the trial day and if accepted an admission meeting will be scheduled.

Does On With Life at New Horizons Center provide transportation services?

No, we do not provide transportation for persons served.

How is this program funded?

Our program is waiver funded, however private pay is available. We will also accept workers’ compensation.
